How to Apply:
- Be a current patient (seen in the last 6 months)
- Submit by May 31, 2025
- IVF treatment must begin before Dec 31, 2025
Because access isn’t optional it’s essential.
Terms & Conditions
- Must be a registered patient at Procreate Fertility Center of Virginia and have been seen in the past 6 months.
- One (1) entry per household using your legal name. Using an alias, initials, or an abbreviated version of your first and last name is prohibited and will be eliminated. More than one entry per household will result in disqualification.
- All entries must be received by Noon PDT on May 31, 2025 (SATURDAY).
- All services must be performed at Procreate Fertility and IVF Virginia and completed by December 31, 2025.
- The winners must have an Antimullerian Hormone (AMH) level greater than or equal to 0.7 ng/mL and age less than 41 years.
- The winners with an Antimullerian Hormone (AMH) less than 0.7 ng/mL or are >41 years of age must be using donor egg through Procreate Fertility.
- Patients must have a medical need for IVF and not have a contraindication to IVF or pregnancy.
- No external services will be covered, e.g., lab work, pre-cycle testing, travel, outside monitoring, donor sperm, fees to genetics lab, etc.
- IVF medications are not included.
- Winning participant(s) for the IVF cycle voucher must be willing to participate in marketing opportunities surrounding this and future contests. By entering the contest you agree to this and will sign a waiver. Please remember to qualify you must:

- This offer does not have any monetary value and is not transferable or refundable for services already paid.
- Winners will be contacted via email and/or phone by June 17, 2025 and must respond within 24 hours. Failure to respond will disqualify entry and a runner up will be contacted.
